All students born on or after January 1, 1957, need to provide proof of immunization or immunity to Measles, Mumps, and Rubella (MMR) and sign the Meningitis Response found on the Health Information Form.Any student born before January 1, 1957, is required to submit proof of age and sign the Meningitis Response.
