Vaccination Portal Update // Dec. 10, 2021
All students taking classes on ground must submit proof of full vaccination against COVID-19 at the MVCC Vaccination Portal.
Students requesting an exemption from the COVID-19 vaccination requirement due to their status as a student registered only for online courses can submit their attestation here.
Students who wish to explore options to move to online schedules are encouraged to contact their Student Support Advisor (SSA). To find the contact info for your SSA, visit mvcc.edu/advisement.

Vaccination Portal // Oct. 9, 2021
With the FDA approval of the Pfizer COVID vaccine as of August 23, 2021, MVCC is requiring full vaccinations for all students enrolled in credit and non-credit on-campus classes.
A “hold” restriction, preventing students from any registration activity (add, drop, new registration, transcript requests, etc.) will be placed for all not in compliance. Students must provide proof of vaccination for campus review by October 8, 2021. Mandatory weekly testing will be required for all students until they are fully vaccinated. Testing for all commuters begins on Monday, September 13, 2021.
SUNY has created a new portal for COVID vaccine proof of immunization. All students completely vaccinated in the state of New York must verify their COVID vaccines utilizing this app. Please visit: HERE and enter your name, date of birth, zip code and phone number that you provided for your vaccine appointment. Note: if you are only attending MVCC you do NOT need to choose a second school from the drop-down menu.
You must be at least two weeks past your final shot for the verification process to be completed.
If you were vaccinated outside of New York (including international students), you must upload a copy of your vaccination record to our protected share-file HERE to be reviewed by the MVCC College RN, Karen Sabonis. International students who obtained a COVID-19 Vaccination may provide documentation signed by a healthcare provider duly licensed to practice medicine in the United States or a foreign country, and such documentation must include the dates of vaccination and name of the COVID-19 Vaccination given. Approved World Health Organization (WHO) vaccines include Pfizer/BioNTech, Astrazeneca-SK Bio, Serum Institute of India, Janssen and Moderna.
All vaccinated students will utilize this app (NY) or share file (out of NYS vaccination) to verify their vaccination.

Frequently Asked Questions (Re: Student Issues Using the Verification App)
A: In order to be verified by your Campus, you will need to log into this application and perform the manual verification by uploading an image of your vaccination record. You will only be able to upload an image of your vaccination record after you attempt to automatically verify your vaccination status by submitting your First Name, Last Name, Date of Birth, Zip Code and Phone Number.
International students who obtained a COVID-19 Vaccination may provide documentation signed by a healthcare provider duly licensed to practice medicine in the United States or a foreign country, and such documentation must include the dates of vaccination and name of the COVID-19 Vaccination given. A: No. Each Campus has designated Campus Representatives who will look at the image to validate the information. If they approve, the image will be deleted, and the Student’s Vaccination Record will be updated to show they are Manually Verified. If it is not validated (either the card does not appear to be legible to read, or there is information missing), then the Campus Representative should reach out to the student to ask them to upload a new image.
Q: I received my final dose, but receive the screen notifying me that I could not be validated. Why?
A: The SUNY Student Vaccination Pass validates your information against what is in the New York State Immunization Information Systems (NYSIIS) and the Citywide Immunization Registry (CIR) for a match. If it has not been more than 15 days since your last vaccination dose, then your information will not be complete in the system and therefore will result in the application stating you could not be found. You need to wait at least 15 days have passed from your last vaccine dose and then try again.
A: The SUNY Student Vaccination Pass validates your information against what is in the New York State Immunization Information Systems (NYSIIS) and the Citywide Immunization Registry (CIR) for a match. You need to make absolutely sure that you have entered your information (First Name, Last Name, Date of Birth, Phone Number, and Permanent Zip code) exactly as you provided on the Vaccination Forms at the time of your appointment! If any of these are different from what you provided, or from what the vaccine provider inputted, at the time of vaccination, then there will be no match found. To correct this, either re-enter your information to match, or you can opt to upload an image of your Vaccination Card and submit for manual verification.
A: The SUNY Student Vaccination Pass allows for your individual campus (along with any other Campuses that you attend for classes) to easily see you have received full vaccination. You are requested to fill out your pass so that is available.
A: No. At present, each registration is good for one year from the final vaccination date recorded. So, if you received your final vaccination on 5/1/2021 and have been verified, then you will not have to re-register until 5/1/2022.
A: Please contact the Excelsior Pass Help Desk at (844) 699-7277 for assistance verifying whether the information you are providing in the SUNY Vaccination Pass is the same information that you provided at the time of your vaccine and for general inquiries/ support regarding data in the New York State Immunization Information Systems (NYSIIS) and the Citywide Immunization Registry (CIR) for a match. You may also contact your campus representative for assistance.
A: Please check your SPAM folder to ensure the Emails are not being flagged as such. Also, check to make sure your Email address entered is correct. If it is not, please updated it on the form.
