Phi Theta Kappa: Forms

Invitations for the Fall 2017 class of inductees were sent on September 15, 2017.

The Fall Induction Ceremony welcoming new members will be held on Wednesday, October 4, 2017 at 3pm in the ACC Commons on the Utica Campus.

To accept this membership invitation, please bring the completed membership application form, induction reservation form, and one-time payment of $70.00 (checks should be payable to "ASC" or "Auxiliary Services Corporation") to Geri Sultenfuss or Steve Frisbee on the Utica Campus (Library, PH203) or to Aubrey Carletta on the Rome Campus (Rome Events and Guest Services).
