Phi Theta Kappa: Forms

Invitations for the Spring 2018 class of inductees were sent on January 23, 2018.

The Spring Induction Ceremony welcoming new members will be held on Wednesday, February 7, 2018 at 3pm in the IT Building Schafer Theater on the Utica Campus.

To accept this membership invitation please bring the completed membership application form, induction reservation form, and one-time payment of $70.00 (checks should be payable to "ASC" or "Auxiliary Services Corporation") to Geri Sultenfuss or Steve Frisbee on the Utica Campus (Library, PH203) or to Aubrey Carletta on the Rome Campus (Student Services Office). You can also follow the directions in your invitation email to join online.

Trip Application Forms

Our chapter is traveling to both the Regional (statewide) Convention and the International Convention this semester. These can be a great opportunity for new members to learn more about the organization and get to know not only our delegation but members from other chapters as well. Below you will find additional information and the applications.
