Phi Theta Kappa: Forms

Invitations for the fall 2015 class of inductees were sent on August 25, 2015.

The Fall Induction Ceremony welcoming new members will be held on Monday, September 14, 2015 at 3pm in the IT Schafer Theater on the Utica Campus.

To accept this membership invitation, please bring the completed membership application form, induction reservation form, and one-time payment of $70.00 (checks should be payable to "ASC" or "Auxiliary Services Corporation") to Geri Sultenfuss or Steve Frisbee on the Utica Campus (Library, PH203) or to Jenny Spinelli on the Rome Campus (Student Services, PC A30).

Dues scholarship application form - needs to be printed and completed (by September 4th to be eligible for the scholarship)
