Phi Theta Kappa: Forms

Invitations for the Spring 2019 class of inductees were sent on January 28, 2019.

The Fall Induction Ceremony welcoming new members will be held on Thursday, February 14, 2019 at 3pm in the IT Building Schafer Theater on the Utica Campus.

To accept this membership invitation please bring the completed membership application form, induction reservation form, and one-time payment of $70.00 (checks should be payable to "ASC" or "Auxiliary Services Corporation") to Geri Sultenfuss or Steve Frisbee on the Utica Campus (Library, PH203) or to Aubrey Carletta on the Rome Campus (Student Services Office). You can also follow the directions in your invitation email to join online.

Trip Application Forms

This semester our chapter is traveling to the Regional (statewide) Convention in Syracuse (March 22nd - 24th) and the International Convention in Orlando, Florida (April 4th - 7th). These can be a great opportunity for new and returning members to learn more about the organization and get to know not only our delegation but members from other chapters as well. Below you will find additional information and the application.
