Phi Theta Kappa: Forms
Phi Theta Kappa Honor Society membership is offered to students who are enrolled at MVCC with a cumulative GPA of 3.5 or higher, and 12 or more credit hours completed which can be applied toward a degree program.
Invitations for the spring 2015 class of inductees were sent on January 9, 2015.
The Spring Induction Ceremony welcoming new members will be held on January 29, 2015 at 3pm in the IT Schafer Theater on the Utica Campus.
To accept this membership invitation, please bring the completed membership application form, induction reservation form, and one-time payment of $70.00 (checks should be payable to "ASC" or "Auxiliary Services Corporation") to Geri Sultenfuss or Steve Frisbee on the Utica Campus (Library, PH203) or to Jenny Spinelli on the Rome Campus (Student Services, PC A30).
Membership application form - needs to be printed and completed (by January 23rd to join the induction ceremony or by May 11th to join this semester)
Induction reservation form - needs to be printed and completed (by January 23rd to join the induction ceremony)
Dues scholarship application form - needs to be printed and completed (by January 22nd to be eligible for the scholarship)
